Psychiatry / evaluation module

The evaluation module¹ is designed as a screening and intervention tool in mental health. If you and your doctor choose this service, a member of our team (comprised of a psychiatrist, a mental health nurse, a psycho-educator and a case manager) will contact you within 48 hours to schedule a personalized phone interview to assess your symptoms, stressors, and social and family context. The mental health team, including the psychiatrist, will then have a discussion to determine the intervention plan. The psychiatrist supervises the evaluation process and reviews your file.

A detailed report will be sent to your doctor ensuring follow-up throughout the process. In certain cases, the psychiatrist will meet you to complete certain aspects of the evaluation.

Psychiatric services¹: Dr. Stéphane Kunicki is an adult psychiatrist with 10 years of experience in a University teaching hospital specializing in affective, anxiety and psychotic disorders, sub specializing in personality disorders.

Home evaluations and rehabilitation²: This team is comprised of a mental health nurse and a psycho-educator with over 50 years of combined experience specializing in at home evaluations of patients and their environment. This service can better guide your doctor and our team in understanding your global situation. They offer rehabilitation services including: community integration, social skills training and one on one support.
